VFPMS medical report template
It is recommended that all regional providers use the VFPMS medical report template when preparing regional medical reports.
It is also recommended that all regional providers who are preparing reports read How to prepare a medico-legal report for VFPMS.

VFPMS regional claims for consultation and medical reports
To make a VFPMS regional claim:
Follow the regional report and invoice process flowchart.
Complete a regional claim form, including service code for examination, date/time patient seen and your bank details for EFT payment.
Please ensure that you use our fee schedule when filling in your regional claim form.
Remit your claim form, medical report and an ATO compliant tax invoice to VFPMS at The Royal Children's Hospital.

Please note: claims must be received within 30 days of consultation.
